The Life Consultant ensures that all members of the interdisciplinary team work together in providing quality services through coordination of essential meetings, advocacy, oversight of medical appointments, provision of supportive counseling, and implementation of specialized person-centered plans. The Life Consultant also maintains relationships with consumers, guardians, team members, professionals, community members, etc. to foster a spirit of cooperation and collaboration for the benefit of the individual.
Life Consultants will have the opportunity to:
Work with a dynamic team of professionals to develop and deliver new and innovative person-centered learning experiences; provide functional skills support to increase independence and promote future success
Assess the unique needs and life goals of each individual and support them as they meet life challenges
Ensure individual’s rights are observed at all times; ensures that individual’s safety and well-being are protected at all times
Provide training and orientations to new and current team members as needed
Conduct regular meetings; maintain an atmosphere conducive to open discussion, team work, integrity, and excellence
Serve as a program liaison to family members, guardians, and the community
· Provide on-call coverage for the Living Enterprise as scheduled or as assigned
The ideal candidate to fill this role has:
Bachelor’s Degree in Social Work, Psychology, Special Education, or a related field.
QIDP Illinois state certified or eligible
One year experience working with individuals with intellectual and other developmental disabilities
Effective oral and written communication skills; ability to write reports and routine business correspondence
Ability to effectively present information and respond to questions from families, employees, and the general public
Ability to work independently and demonstrate good problem solving skills
Ability to perform in a professional manner and embody the Values of Aspire at all times
Valid Driver’s License with good driving record; ability to maintain PACE certification
Ability to work On-Call position with flexible hours Monday through Friday, including occasional weekends and some holidays
