We have an exciting opportunity to join our team as a Community Health Rep.
In this role, the successful candidate The New York University School of Medicine Department of Population Health is seeking a part-time Community Health Worker (CHW) to work with patients at-risk of developing Type 2 diabetes.
CHWs will be conducting outreach to patients at either Bellevue Hospital Center or the VA NY Harbor Health Care System - Manhattan Hospital who are at-risk of developing diabetes. CHWs will encourage patients to set health-related goals, conduct one-on-one and group-based health education sessions to promote weight loss and encourage healthful eating and exercise, coordinate patient care with the clinical providers and staff, and refer patients to community-based diabetes prevention programs.
Successful candidates for this position will have experience working in a community setting directly with community members. They will be passionate about community health issues as well as working one-on-one with patients to promote health and wellness.
Attend weekly case review meetings where they will discuss challenging cases, CHW best practices, and undergo continual training and professional development.
Report to Program Manager and Research Coordinator
Collaborate with patients' clinical team at patient's hospital
Address patients' clinical or social needs by directing them to appropriate services
Conduct group health education sessions with multiple patients (eg, visiting farmers' markets, group walks)
Provide individual counseling and support; assist participant with goal-setting and lifestyle changes
Conduct health education sessions (over the phone, in-person, via text) on diabetes prevention strategies
Link patients with diabetes prevention health program in the community
Track outreach activities and patient progress in a project database
Establish trusting relationships with patients
Conduct outreach phone calls and in-person sessions with patients
To qualify you must have a Minimum of high school education/GED required, bachelor's degree preferred but not required.
Military veteran/experience working with military/veteran population
Minimum 1 year of experience in community-based clinical or social service delivery.
Basic computer knowledge/skills required.
Good advocacy and community organizing skills and/or experience with community leadership activities
Warm, friendly, open, energetic, empathetic
Strong organizational skills
Ability to manage time and priorities
Works well with a wide range of people
Qualified candidates must be able to effectively communicate with all levels of the organization.
