Under the direction of the Program Manager and Medical Director, the Clinical Navigator will assist back pain patients and their caregivers on their journey through the back wellness program as an advocate, educator, coordinator and clinical liaison. Through a concierge approach, the navigator will coordinate scheduling of the potential patient for assessments, second opinions, testing, etc. at the appropriate location within the region. The navigator will serve as primary point of contact for the region and is responsible for coordinating the care and services of the identified patient population for an extended period of time across the continuum of care. He/she will promote effective resource utilization and assume a leadership role with the interdisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers. The navigator refers patients/families to appropriate programs, implements tools, proposes care and clinical pathways, and assists in marketing the Centura Spine Care program. The navigator will participate in outcome data collection throughout the patient's program and follow up. For operational, procedural, clinical and personnel issues, this position reports to the Program Manager of Penrose-St. Francis Health Services.
MINIMUM EDUCATION REQUIREMENTS
Graduate from a NLN or AACN Accredited nursing program.
Registered Nurse with Bachelor's Degree preferred
MINIMUM EXPERIENCE REQUIREMENTS
Must have 3-5 years relevant healthcare experience
Minimum of 2-3 years of experience in caring for the spine patient population in the inpatient and/or outpatient setting
Experience/knowledge in community health setting/services desirable
Experience with data management preferred
Current RN license to practice in the state of Colorado
BLS for healthcare provider's certification
Orthopedic nurse certification (ONC), Certified Neuroscience Registered Nurse (CNRN) or Certified Rehabilitation Registered Nurse (CRRN) preferred (or obtain within 1 year of employment)
Sedentary Work - prolonged periods of sitting and exert/lift up to 10 lbs. force occasionally.
Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly.
