Posting Summary: The Department of Medicine, Division of Infectious Diseases, seeks a CHARLI Health Educator/Medical Case Manager to assist with the Ryan White Program. The incumbent serves as contact for HIV/AIDS clinic patients to ensure access to medical services, support services, and educational resources. The incumbent determines client's eligibility for programs provided. This position is responsible for providing a full continuum of HIV services to include pre-release HIV prevention education, HIV testing, and linkages to care for inmates 60 days prior to release in Virginia correctional facilities such as federal, state prisons, regional, local and city. Post release case management services can include but are not limited to services that ensure retention in care and adherence, referrals to service providers and primary prevention interventions for people living with HIV.
Qualified applicants must have a Bachelor's degree in higher education administration, health promotion, counseling, or related field of study. Applicant must have at least four years of experience in a professional field. Experience in program/event planning and evaluation in a state or federal corrections facility is typical. Applicant should have a minimum of 2 years' experience in assisting with patient services, coordination of medical care, assistance with insurance and medication access which in includes correctional facilities. RN, LPN, MPH, BSW, or MSW and experience working with indigent/corrections client populations as well as knowledge of HIV/AIDS are preferred.
