The Clinical Social Worker in our Diabetes Day Treatment Program will be responsible for:
Providing a range of clinical social work services and case management to new diagnosis in the Diabetes Day Treatment Program and Diabetes inpatient.
Performing psychosocial screening and assessment, counseling, crisis intervention, facilitation of use of hospital and community services, resource information and referral, program development and psychosocial consultation to multidisciplinary team.
To qualify, you must have:
A Master's degree in Social Work, LICSW required, with a minimum of 2 years of Social Work experience.
Flexibility and the ability to work in fast-paced, dynamic environment.
Excellent communication skills.
Experience in medical environment, bereavement, child protection issues highly preferred.

Schedule : Every other Sat and Sun 7 hours per day 8:30AM - 4PM
