The public health nurse education specialist is responsible for providing in home, family centered education and public health prevention and early intervention services using the Parents As Teachers (PAT) curriculum. The specialist promotes positive parenting behaviors, healthy children, school readiness, and parent/family well-being. The specialist facilitates monthly family support and community based learning sessions (“group connections”). She/he actively contributes to all required reports to funders, as well as data collection and analyses for enrolled families and prepares required reports as needed. the specialist participates in required pat and related trainings for certification as well as required professional development education and skill building. Advanced skills in motivational interviewing, mandated documentation and data base systems are essential components of the position. She/he reports directly to the Mabel Morris family home visitor supervisor and the nurse administrator of the Philadelphia Nurse-Family Partnership (NFP) and Mabel Morris (MM) Family Home Visit Program.
RESPONSIBILITIES: Works with low income, high risk population utilizing the Parents As Teachers curriculum and home visitation model. Attains certification as PAT Specialist. Conducts community outreach for program enrollment. Enrolls eligible families and has minimum caseload of 15 families with approximately 30 eligible children (0-5 years) in the household. Makes weekly to monthly home visit with each family or as needed to help parents create positive learning environments that support their child’s development and school readiness.Conducts parent and child assessments including environmental health, mental health, child ages and stages screenings. Adheres to the required enrollment, screening and referral processes identified by PAT. Plans and facilitates monthly family workshops (Group Connections). Develops family partnership agreements. Utilizes required management information systems including Penelope, Visit Tracker and Apricot as well as state systems. Participates in weekly one-on-one reflective supervision sessions. Participates in regularly scheduled clinical conferences and educational sessions. Advocates on behalf of vulnerable families at risk for homelessness, unemployment, loss of benefits and other matters of concern. Collaborative works with others regarding the Quality Endorsement and Improvement Review, the Performance Review rating instruments and federal evaluations. Maintains open communication with colleagues, clients, supervisors and community at large regarding program services and client needs.

EXPERIENCE: Minimum of two years’ experience working with vulnerable, urban families and/or educational preparation in public health/community nursing with relevant clinical coursework.
About Philadelphia Nurse Family Partnership and Mabel Morris Family Home Visit Programs
Mabel Morris Family Home Visit Program (MM) is an evidence based program for low-income parents in Philadelphia, raising children under five. The program is an Affiliate of the Parents as Teachers evidence based home visiting model that provides activities, handouts and the foundation for each home visit. The MM program provides information, support and encouragement parents need to help their chi...ldren develop optimally during the crucial early years of life. As a home-based program, we provide visits to families every other week or more frequently depending on the client’s needs. Visits are based around three areas of emphasis: Parent child interaction, development-centered parenting, and family well-being. Family-centered assessment and goal setting accompany the model’s four interrelated and integrated components: personal visits, group connections, screening, and resource network. Together, these form a dynamic package of services.
The program is named in honor of Ms. Mabel Morris, a woman who stood amongst the first public health nurse leaders in Philadelphia. Morris served the community for many decades before her passing in 2009. Morris’ philosophy, “Knowledge is power and information is how to get it,” informs the vision of our program: all children will learn, grow and develop to realize their full potential. Eligibility includes pregnant and parenting women/parents with children up to five years of age. Income criterion is 250% of poverty or less. Families must reside in Philadelphia and voluntarily enroll in program.
